2. READ FIRST READ THE GUIDELINES VERY CAREFULLY. ALL DOCUMENTARY, GRADE AND PROGRAM-SPECIFIC REQUIREMENTS ARE AVAILABLE IN THIS AREA.
3. CONFIRMATION CLICK THE CHOICES THAT ARE SUITABLE FOR YOUR APPLICATION. *THIS APPLICATION IS APPLICABLE TO ALL INCOMING FIRST YEAR APPLICANTS.

6. FORM PREVIEW: ENSURE THAT ALL INFORMATIONS ARE TRUE AND CORRECT BEFORE SUBMITTING THE APPLICATION. INTEGRITY OF APPLICATION THE APPLICANT GUARANTEES THE COMPLETENESS AND ACCURACY OF INFORMATION DISCLOSED OR CONTAINED IN DOCUMENTS SUBMITTED. EVALUATION
WHEN THE APPLICATION FORM IS SUBMITTED IT WILL BE EVALUATED. WAIT UNTIL IT APPROVED AND THE TEST PERMIT HAS BEEN GIVEN
EXAM
1. GO TO THE DESIGNATED TESTING CENTER ON THE DATE SPECIFIED IN THE ADMISSION TEST PERMIT. BE AT UNIVERSITY 30 MINUTES BEFORE THE SCHEDULED TIME. CHECK THE ADMISSION TEST PERMIT TO ENSURE THE CORRECT SCHEDULE DATE, TIME, AND VENUE.
2. THE UNIVERSITY IMPLEMENTS A NO TEST PERMIT, NO TEST POLICY
3. BRING ONLY THE TEST PERMIT, ANY VALID ID, TWO GOOD QUALITY PENCILS, A SHARPENER, AND A RUBBER ERASER.
4. THE USE OF CELLPHONES AND/OR CALCULATING DEVICES DURING THE TEST IS STRICTLY PROHIBITED AND CAN BE CAUSE FOR DISQUALIFICATION.
5. ONSITE APPLICATION IS NOT ALLOWED ON THE DAY OF TESTING

PRE-REGISTRATION
1. CLICK "PRE-REGISTRATION" AND CONFIRM YOUR PRE-REGISTRATION TO SIGNIFY YOUR INTENT TO ENROLL.
2. CLICK "MEDICAL REQUEST SLIP" UNDER ADMISSION CHECK LIST TO PRINT MEDICAL REQUEST SLIP AND COMPLY WITH THE REQUIREMENTS.
3. ALL REQUIREMENTS MUST BE DONE BY DEPARTMENT OF HEALTH ACCREDITED FACILITIES.
4. CLICK "STUDENT PORTAL" TO SECURE YOUR STUDENT PORTAL LOG IN CREDENTIALS.
5. CLICK "APPOINTMENT" UNDER ADMISSION CHECK LIST IF THE MEDICAL REQUIREMENTS ARE COMPLETE.
6. CREATE YOUR APPOINTMENT SCHEDULE FOR THE SUBMISSION OF MEDICAL REQUIREMENTS AND MEDICAL EXAMINATION. PROCEED TO THE HEALTH SERVICES OFFICE OF YOUR CAMPUS ON THE SCHEDULED DATE FOR THE PRE-ENROLMENT MEDICAL AND DENTAL EXAMINATION AND SUBMISSION OF MEDICAL REQUIREMENTS.
BRING THE FOLLOWING DOCUMENTS:
- NOTICE OF ADMISSION
- CHEST X-RAY RESULT RESULT (WITHIN SIX MONTHS VALIDITY)
- LABORATORY BLOOD TYPE RESULT OR ANY ID OR DOCUMENT/S THAT INDICATES BLOOD TYPE OF THE STUDENT
- DRUG TEST RESULT (WITHIN ONE YEAR VALIDITY)
- PRINTED DATE OF APPOINTMENT
7. CLICK "APPOINTMENT" UNDER ADMISSION CHECK LIST FOR THE SCHEDULE OF THE SUBMISSION OF ENROLLMENT REQUIREMENTS TO THE REGISTRATION SERVICES OFFICE.
NOTE: ONLY STUDENTS WHO HAVE UNDERGONE MEDICAL EXAMINATION SHALL MAKE AN APPOINTMENT WITH THE REGISTRATION SERVICES OFFICE.
8. SUBMIT THE COMPLETE ENROLLMENT REQUIREMENTS TO THE REGISTRATION SERVICES OFFICE ON THE APPOINTMENT DATE YOU HAVE SELECTED. BRING THE FOLLOWING DOCUMENTS:
- NOTICE OF ADMISSION (NOA) FROM TAO (WITH STAMP FROM THE HEALTH SERVICES - ORIGINAL COPY)
- FORM 138 / GRADE 12 REPORT CARD (FIRST AND SECOND SEMESTER ORIGINAL COPY)
- NOTE: PROVIDE A PHOTOCOPY OF THE FORM 138/GRADE 12 REPORT CARD FOR SCHOLARSHIP OR OTHER PURPOSES BEFORE SUBMITTING THE ORIGINAL COPY
- PSA/NSO BIRTH CERTIFICATE /MARRIAGE CONTRACT FOR MARRIED FEMALE ENROLLEE (CLEAR COPY, BRING THE ORIGINAL COPY FOR VERIFICATION)
- NOTE: PROVIDE ANY OF THE FOLLOWING SUPPORTING DOCUMENTS ISSUED BY THE LOCAL CIVIL REGISTRY IN CASE NAMES ARE BLURRED OR UNREADABLE
- CERTIFICATE OF LIVE BIRTH
- CERTIFICATION OF BIRTH
- CERTIFICATE OF GOOD MORAL CHARACTER (ORIGINAL COPY)
- FORM 137 WITH REMARKS, “COPY FOR BATANGAS STATE UNIVERSITY” AND WITH DATE OF GRADUATION AND LEARNER REFERENCE NUMBER (LRN).
9. NOTE: FORM 137 MUST BE SUBMITTED BY THE STUDENT OR THE SHS TO THE REGISTRATION SERVICES OFFICE ON OR BEFORE THE OPENING OF CLASSES. THIS IS NOT IMMEDIATELY REQUIRED AS THIS WILL NEED A REQUEST FROM BATSTATEU TO YOUR SCHOOL.
10. LOG-IN TO YOUR STUDENT PORTAL TO CHECK THE ENROLLMENT DETAILS SUCH AS ENROLLMENT STATUS, COURSES ENROLLED AND SCHEDULES.
